SRYBA requires that all organizations requesting a donation complete this form to be presented to our Board.
Please submit the completed form to our Board President prior to the meeting and then attend our monthly board meeting (typically the last Sunday of each month - see calendar) where you will be allowed up to 10 minutes to present your request.
